Main duties of the job

We are looking for an experienced, proactive and enthusiastic individual with a proven track record in a well performing environment to join our friendly GP training practice as a Practice Nurse with an interest in Diabetes or a Nursing Associate. Our clinical team consists of 3 GP’s, 1 GP Registrar, 1 Nurse Practitioner, 1 Nursing Associate, 1 Healthcare Assistant and 1 Phlebotomist.

You must have a deep understanding of health services together with a passion for improving lives and improving healthcare. This is a great time for individuals seeking a fulfilling and interactive role to join us and develop their career.

The post-holder duties will include all tasks normally undertaken by an experienced RGN working in primary care. The post-holder will be expected to provide a high standard of nursing care for patient’s and provide nursing assistance to doctors and other members of the primary healthcare team, and support work that maximises Practice income (eg QOF and enhanced service contracts). There will be some additional specialist clinical duties and responsibilities, for example diabetes and anticoagulation INR.

Conducting and recording core clinical observations: Venepuncture and phlebotomy. Venepuncture and phlebotomy, Urinalysis, Conducting pathology tests, for example urine culture, swabs, phlebotomy, Blood pressure, Height / weight / BMI, ECG. Basic wound and ulcer care and dressings. Removal of sutures. Cervical smears.

Basic contraception advice. Basic healthy lifestyle advice and referral onwards. Supporting carers access support. Ensure vaccines are administered under patient group directions.

Routine immunisations (eg Covid, influenza, pneumococcal, shingles). Infant immunisation and vaccinations. Chaperoning and assisting patients where appropriate who are being examined by another clinician.

Able to recognize and manage anaphylaxis according to current UK guidelines. Able to perform Cardio-pulmonary resuscitation according to current UK guidelines. Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs. Ensure safety netting advice given to patients, make sure patients know what to do if their symptoms worsen, and record this.

Able to document consultations according to NMC guidelines. Housekeeping, ordering and infection control duties including: Maintaining general tidiness and cleanliness of nurses and treatment rooms. Maintaining and cleaning equipment and fridges. Maintaining GP and Nurses rooms, stocking and rotating items as required. Ordering of drugs and stock. Using our SystmOne clinical system

JOB TITLE:   PRACTICE NURSE

REPORTS TO:                    SENIOR PRACTICE NURSE / THE PARTNERS (Clinically)

SALARY                                DEPENDANT ON QUALIFICATIONS AND EXPERIENCE

Job Summary:

To the assist medical personnel in the care of Practice patients to include treatment, preventative care, screening and patient education.

Job Responsibilities:

  • Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols.
  • Provide general and specific health screenings to the Practice patients, including those with long term conditions (within agreed protocols) with referral to senior Nurse / general practitioners as necessary.
  • Maintain skills and an interest in Diabetes, Asthma, COPD or Coronary Heart Disease, Hypertension, Stroke/TIA and Contraception.
  • Assist in and perform routine tasks related to patient care as directed by senior nurse and GPs
  • Wound care / Ulcer care / Doppler
  • Removal of Sutures
  • New Patient Medicals / Urinalysis/NHS Health Checks
  • ECGs
  • Warfarin Monitoring (INR)
  • Travel Clinic, to include vaccinations, medications and advice
  • Routine immunisations / Childhood immunisations according to British schedule.
  • Undertake the collection of pathological specimens including intravenous blood samples, swabs, cervical smears etc and ensure cleanliness of any equipment used.
  • Perform an investigatory procedures requested by the General Practitioners.

Other tasks will include:

  • Maintaining and cleaning equipment used by the nurses and GPs
  • Maintaining Nurses rooms, stocking and rotating items as required
  • Maintain general tidiness and cleanliness of nurses and treatment rooms
  • Assisting with the ordering of vaccinations to maintain stock levels
  • Chaperoning and assisting patients where appropriate who are being examined by another clinician
  • A duty to advise senior nurses of potential problems or errors within the range of assigned tasks
  • Attend and participate in any Practice meetings when required.
  • A requirement to undertake home visits
  • Any other delegated duties appropriate to the post

Administrative and professional responsibilities

  • Participate in the administrative and professional responsibilities of the Practice team.
  • Ensure accurate notes of all consultations and treatments are recorded in the patients’ notes on the clinical computer system as appropriate.
  • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice.
  • Ensure collection and maintenance of statistical information required for regular and ad hoc reports.
  • Assist in the routine recall of patients with chronic diseases.
  • Attend and participate in Practice meetings as required.
  • Assist in the formulation of Practice philosophy, strategy and policy and develop appropriate protocols.

Student/staff education and training

  • Participate in the education and training of students of all disciplines and the induction of all members of Practice staff.

Research projects

  • Co-operate and participate as required in any research projects within the Practice.

  • In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.   They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately

Job summary

We have an exciting opportunity for a Nursing Associate to join our Practice Nursing team. The Nursing Associate will work under the supervision of the Practice Nursing Team. Leadership and support will be provided by the Nursing Lead and GP Partners. Nursing Associates are equipped with the knowledge, understanding, skills, attitudes, and behaviours to support general practice. The Nursing Associate will assist with the delivery of health promotion, phlebotomy, chronic disease management, immunisation and vaccinations and other clinical roles in line with their training and level of experience.

Main duties of the job

· Work within the Practice to provide and monitor care, contributing to the delivery of excellent patient care, supporting the existing workforce under direct or indirect supervision

· Improve safety and quality of care at every opportunity

· Undertake additional clinical training as required by the Practice

· Support registered nurses and the wider team to enable them to focus on the more complex clinical care

· To support with chronic disease clinics and maintaining registers, deliver patient care, referring patients to other members of the Practice or to secondary/specialist care as appropriate. To implement and evaluate individual treatment plans dependant on training and experience

Job responsibilities

· Accurately perform and record clinical observations such as blood pressure, temperature, respiration, and pulse rates

· Engage positively with patients/carers/families and staff members promoting mutually respectful and appropriate relationships

· Communicate effectively with patients regarding providing and planning their care and carrying out risk assessment needs as required

· Communicate complex and sensitive information verbally and in writing. Adapt and tailor communication skills to the need of the listener

· To act as patient advocate during practice meetings, sharing any relevant information confidently and accurately, maintaining patient confidentiality appropriately

· Develop an understanding of MDT structures and members roles

· Maintain a good working knowledge of Health and Safety procedures, fire safety and mandatory training as per practice policies. Participate in policy development and data collection where appropriate

· Work flexibly to meet the needs of the service and patients. Adapt to what may be potentially challenging and changing situations at times

· Willing to undertake other duties deemed appropriate to a Nursing Associate following correct training/guidance and agreed by the post holder and their line manager
